London's Royal Docks Dive Permit

a.	All diving projects within the RoDMA lease area are to be in accordance with the Diving at Work Regulations 1997.
b.	The Diving Contractor shall ensure, so far as is reasonably practicable, that the diving project is planned, managed, and conducted in a manner which protects the health and safety of all persons taking part in that project. 
c.	The Diving Contractor shall ensure, so far as reasonably practicable, that any person taking part in the diving project complies with the requirements and prohibitions imposed on him by or under the relevant statutory provisions and observes the provisions of the Diving Project Plan; ensure that a record containing the required particulars is kept for each diving operation.
d.	The Diving Contractor has supplied the appropriate information to the Health &amp; Safety Executive and provided evidence to RoDMA to confirm all supervisors are appropriately qualified and all divers are qualified and medically fit to dive.
e.	A full risk assessment has been undertaken to identify site-specific hazards and their risks, and a diving plan prepared.
f.	RoDMA has supplied the Diving Contractor with the results of any risk assessments undertaken that could affect the health and safety of the diving team, identified all known underwater or over-water items which might cause a hazard to the diving team, considered whether any other activities in the vicinity may affect the safety of the diving project, provided details of any possible substances likely to be encountered, and will notify any changes to this information.
g.	RoDMA will provide facilities and all reasonable support in the event of an emergency.
h.	The Diving Contractor will at all times observe and apply the provisions of his safety policy and diving plan during the execution of the project.
i.	The Diving Contractor will at all times permit the Dockmaster or his authorised representative to enter upon the site of the diving project at any time during working hours and have free and unfettered access to each and every part of the area of the work.
j.	The Dockmaster may carry out periodic audits of the dive site to check the Diving Contractor is complying with the Regulations, any additional RoDMA requirement and the Diving Plan.
k.	The Diving Supervisor will co-ordinate diving activities in the diving project with the RoDMA Dockmaster tel: 07711 300132, whose proper operational instructions must be complied with at all times. The Dockmaster is to be notified at the commencement and completion of each diving day by VHF Channel 72 (KGV Control) or mobile and copies of the Daily Diary Record Sheets are to be supplied at the end of each week.
l.	The Diving Supervisor is to inform the Port of London Authority, Vessel Traffic Services (VTS) 0208 855 0315 each day they are diving and the location in which they are operating. 
m.	All incidents (accidents, near misses or dangerous occurrences) which occur during diving operations are to be reported to the Dock Master.
n.	During inspections the Dockmaster will notify the supervisor initially orally and subsequently in writing of any method of operation or other item which in his opinion falls below the statutory requirements of the Regulations or safe system of work. Failure to take the appropriate remedial action will result in the immediate termination of diving operations and determination of this Permit.
